What it is

The protocol as data: define a plan once — HbA1c at day 90, foot exam at day 180 — and enrolling a patient materialises dated milestones in one transaction. The due list is the daily driver; a milestone is done, or skipped with a recorded reason, or loudly overdue. Programmes: patients on long-term steroids, whoever prescribed them, with the monitoring checks on a plan from your library, and for a steroid-dependent patient the sick-day rules in the prescriber’s own words, recorded as explained, then a numbered emergency card carrying exactly those words (a changed rule needs explaining again and a new card); transplant recipients with each immunosuppressant level read from the lab against the range the transplant team set, a level outside it listed until the team is noted as having seen it, and surveillance biopsies on the transplant team’s own protocol for the organ (the months after transplant, with its source; none ships) — each protocol biopsy dated from the transplant as done, overdue or upcoming, and every biopsy, protocol or for cause, kept with the grading system as the pathologist names it, the grade and whether rejection was reported, in date order beside the one before so a new grade is read against the trend; quit plans for tobacco with the Fagerström score as scored, the quit date, counselling and NRT or a medicine as prescribed, and follow-up calls on your own call schedule; and oral care rounds for residents who cannot clean their own teeth, where anything seen opens a dentist referral kept until its outcome.

How it works

  1. Define the plan onceName it and list its milestones as day offsets — HbA1c at day 90, foot exam at day 180. The protocol becomes data instead of a poster in the corridor.
  2. Enrol the patientEvery milestone the plan owes them is created as a dated row in one go, against that patient.
  3. Work the due listThe next fourteen days, overdue at the top. That is the screen the team opens in the morning, and it is the whole of the app’s daily job.
  4. Close each one honestly, or stop the planDone, or skipped with a reason the plan keeps. There is no reschedule verb, so a missed milestone stays overdue until somebody closes it. A plan that stops fitting is stopped with a reason; closed milestones stay readable.
  5. Run a programmeOn Programmes, pick long-term steroids, transplant levels and biopsies, quitting tobacco or oral care rounds. Steroids: the prescriber writes a dependent patient’s sick-day rules in their own words, someone records them explained, then the numbered emergency card carries them; checks ride a plan from your library. Transplants: each level from the lab against the range the transplant team set, and surveillance biopsies due on the team’s own protocol with every grade as reported beside the one before. Tobacco: the quit date, support as prescribed and calls on your own schedule. Oral care: the rounds a day, and anything seen opens a dentist referral until its outcome.

Rules it keeps

What it does not do, by design. A listing with only benefits is an advertisement.

A missed milestone is never silently rescheduled — there is no reschedule verb in the schema.
It stays overdue until someone closes it honestly.
It writes no sick-day dose, transplant range, biopsy protocol or call schedule of its own: each is the clinician’s or the clinic’s, and the dose stays the prescriber’s.
It does not grade a biopsy or reproduce a grading system: the grade is the pathologist’s, as reported.

What data does CarePlans read and write?
CarePlans reads the chart, plans, lab results and writes plan instances, milestones, steroid register and emergency cards, transplant ranges, transplant biopsy protocol and biopsy grades, quit plans and calls, oral care rounds and dentist referrals. Its data class is Full PHI. Every app works on the same patient record — nothing is copied into a silo.
What does CarePlans deliberately not do?
