What DementiaCare does

MMSE/MoCA on cadence, functional staging, caregiver burden scores with support referrals, wandering-risk plans, and advance-care conversations documented while they still can be.

How you actually use it

  1. No caregiver contact, no programThe enrolment takes the diagnosis — Alzheimer’s, vascular, Lewy body, frontotemporal, mixed or unspecified — with a caregiver’s name and phone; the schema refuses the row without them, which is what the card’s title says.
  2. Stage the cognitionAn MMSE on 0–30, a CDR of 0, 0.5, 1, 2 or 3, or both; an entry with neither is refused, and an MMSE off the scale is refused as a scoring error.
  3. Score the caregiver’s burdenThe Zarit Burden Interview totals 0–88 and anything outside that is refused; the band the registry shows is computed from the latest score rather than stored.
  4. Review safetyWandering risk low, medium or high, with ticks for still driving and meds locked away; the latest review is the safety state, and a review older than 180 days — or none at all — reads as safety review due.
  5. Change the caregiver as it changesThe caregiver’s name and phone can be rewritten, but only on an active enrolment; Stop asks why the patient is leaving dementia care.
