What it is

MMSE/MoCA on cadence, functional staging, caregiver burden scores with support referrals, wandering-risk plans, and advance-care conversations documented while they still can be. Enrolments and milestones are kept on Care Plans, which installs with it. A resident’s life story is on file for any staff member on any shift: once the resident or family consents to share it with staff, each section — career, family and the people who matter, places, what they loved, routines, what upsets them, the names they like to be called, faith if the family chooses — is kept in their words with who told it and when, every revision kept beside the last; a withdrawn consent hides it. The behaviour log keeps each episode as observed — when, where, what happened, the trigger as staff saw it, what was tried and how each went, any injury, a named scale’s score as scored — and counts the pattern by hour, weekday, trigger and category for a clinician to review with a note.

How it works

No caregiver contact, no programThe enrolment takes the diagnosis — Alzheimer’s, vascular, Lewy body, frontotemporal, mixed or unspecified — with a caregiver’s name and phone; the schema refuses the row without them. The caregiver can be changed on an active enrolment, and Stop asks why the patient is leaving dementia care.
Stage the cognitionAn MMSE on 0–30, a CDR of 0, 0.5, 1, 2 or 3, or both; an entry with neither is refused, and an MMSE off the scale is refused as a scoring error.
Score the burden and review safetyThe Zarit Burden Interview totals 0–88, and the band the registry shows is computed from the latest score. Safety is wandering risk low, medium or high with ticks for still driving and meds locked away; the latest review is the safety state, and one older than 180 days — or none — reads as due.
Keep the life story staff use on every shiftOnce the resident or family consents to share it with staff, each section — career, family, places, what they loved, routines, what upsets them, the names they like to be called, faith if the family chooses — is kept in their words with who told it, their relationship and the day. A revision never overwrites the last; a withdrawn consent hides the story. It is not a clinical plan.
Log behaviour episodes and review the patternEach episode as observed: when, where, what happened, the trigger as staff saw it, what was tried and how each went, any injury, a named scale’s score as scored; corrections are addenda. The Behaviour log counts episodes by hour, weekday, trigger and category, and a prescribing clinician reviews the pattern with a note — the app suggests no response and no medicine.
