Follow-up that reaches a 2G feature phone, in the patient’s language — and the doctor’s written instructions as a voice note in it.
Post-discharge and chronic check-ins over IVR voice calls, SMS and WhatsApp — no app, no literacy assumption. Structured question scripts with worrying answers escalated to a nurse worklist under response clocks, and follow-up visits booked before problems become readmissions. A scheduled call is placed from a phone line’s calling account, entered on Phone Calls, and how it ended and what the patient pressed come back onto the job as its attempt; a line with no account places nothing and says so. For a patient who cannot read the printed sheet, staff record the written discharge or prescription instruction in the patient’s language and dialect, with the written words it speaks and who spoke it; a clinician listens and approves it before it reaches the Patient App, and every play is counted.

Read out of the code, not the brochure: each app below is here because one service queries the other’s tables. Install either side and the hand-over is already wired.
