What HeartFailureHome does

Daily home weights with two-kilogram rules, symptom check-ins, K/creatinine after every titration, and deterioration flags that reach the team before the emergency department does.

How you actually use it

  1. Enrol with the dry weightFind the patient, enter the dry weight in kilograms and Enrol. The alert threshold defaults to 2 kg over 48 hours; Targets changes both per patient.
  2. Record the weightToday’s kilograms. The service keeps one weight per day and refuses an implausible one.
  3. Check inNot breathless, breathless on exertion or breathless at rest, with swelling and orthopnoea ticked as they are.
  4. Read the morning call listWeight alerts is every patient whose 48-hour gain has crossed their own threshold, computed on every load and never stored.
  5. Log the diuretic adjustmentDrug, dose and reason, append-only and authored. The rule fires; the clinician decides.
