What it is

Insulin, anticoagulant and antihypertensive titration as worklists — incoming readings queue the decision to the right clinician with the protocol beside the trend. The INR diary gives each AnticoagClinic patient one running diary: every clinic INR with the target it was read against, the dose the clinic decided after it in the clinic’s words and the next test date — read from the clinic, never copied — with the patient’s own entries in between: a next test booked, a home test from their own meter (always marked as the patient’s meter, never a clinic result) and what they say they took. It prints as a plain-words diary card the patient keeps.

How it works

Read the board and its sourcesEvery row is computed live from the condition program that owns the reading, with the basis — the readings that put it there — and every source is named, with a programme this clinic never installed listed as not read rather than silently omitted.
Decide, and say on whatIncrease, decrease, hold, switch, refer or no change — the first three ask for detail — and the decision is stored with the basis it was made on, which cannot be blank.
Understand what deciding does not doA decision changes no dose, target, band or reading anywhere; it records that a named clinician looked, which is what takes the row off the board.
Snooze what you cannot act on todayA snooze takes a date and a reason — ‘later’ is refused — and the row comes back by itself when that date passes.
Keep each INR patient’s diaryThe INR diary tab reads every AnticoagClinic result, its target and the dose decided after it, adds what the patient reports — a test booked, a home meter reading marked as theirs, what they took — and prints the diary as a card for them to keep; it changes no dose.
