What it is

Every implanted device — joint, mesh, pacemaker, lens — recorded with UDI, batch and site against the patient and the case. A manufacturer recall becomes a patient list in seconds, with outreach through consented channels. Retrieval windows: a device meant to come out — a temporary inferior vena cava filter first among them — is watched from the day it went in. The clinician sets the window (with where it came from), who should take it out and how often the team is reminded; nothing is filled in. The list puts a passed window and a reminder due at the top, each reminder logged restarts the schedule, and the watch ends only when the device is recorded out on the registry or a clinician writes down why it is staying. Hernia: every hernia repair is recorded against the same fields whether open, laparoscopic or robotic — the hernia, the repair as the surgeon names it, the mesh read off its registry record with its lot (or named as recorded) — and followed up for recurrence on the surgeon’s own dates; a recurrence stays on the original repair, so recurrence reads back by technique and by mesh, and a re-repair names the repair that recurred. The record stays with the clinic; nothing is sent to a national registry.
