What MaxFax does

One case sheet per presentation, of one kind. The trauma sheet is a fracture register: each fracture by site and pattern, the fixation that held it, and every plate and screw scanned onto the hospital’s implant register under the same surgical case — so a recall on a lot number finds the jaw it went into. The orthognathic sheet carries the diagnosis, the osteotomies, the movements in millimetres, the splint and the four dates; the TMJ sheet the opening, the deviation, the click, the lock and what was decided. Dental status is charted on the same tooth chart the dental chair uses. A sheet is listed for surgery onto the surgical record and, with a theatre named, onto that theatre’s list. It is signed by the surgeon on their own verified registration, and the database refuses any other signature.

How you actually use it

1. 1Open a sheet for the presentationPick the patient, say whether it is trauma, orthognathic or TMJ, and write the one line the list will show.
2. 2Record what you foundOn a trauma sheet, each fracture by site and pattern. On an orthognathic sheet, the osteotomies and the movements. On a TMJ sheet, the opening, the sounds and the decision.
3. 3List it for surgeryThe case goes on the surgical record, where fitness and consent are stamped; name a theatre and it is on that theatre’s list too.
4. 4Scan every plate and screwEach device goes on the implant register under this patient and this case. A removal is recorded against the placement, with the reason.
5. 5Sign itOn your own verified registration. A signed sheet is frozen; nothing more can be added to it.

What it does not do

It keeps no second hardware table — a plate is an implant on the implant register, and its removal is that register’s explant. It keeps no second tooth chart and no second theatre list. It does not sign a sheet for anybody but the person signing it, and not on a qualification, an unverified number or a lapsed one.
