What it is

Every patient admitted is screened for malnutrition with a published tool, MUST or NRS-2002, scored from the tool’s own parts on screen: the BMI, weight-loss and acute-disease steps of MUST, or the nutritional status, severity of disease and age of NRS-2002. The total and the band are worked out with the cut-offs the tools publish (MUST 2 or more high; NRS-2002 3 or more at risk), both cited. When the band says at risk, the screener refers the patient to the dietitian or records why not; the dietitian sees each referral on their own list and records what they did. The ward list puts first the patients never screened, then the ones at risk with no referral, then rescreens due on the clinic’s own interval.
