What it is

Ambient documentation through the AI gateway: the consult drafted into a structured note with suggested codes, presented as a draft the doctor edits and signs — never auto-finalised, every token audited, Indian accents and code-switching handled as first-class, consent captured before any recording. One note, filled three ways and switchable at any moment: type a field, tap its microphone to dictate just that field, or let the recording fill the fields still empty — each field marked typed, dictated or AI, and an AI suggestion waits beside its field until the doctor uses it or adds it to their own words; nothing typed is overwritten.

How it works

  1. Open the chart from the day’s queueThe scribe is the Consultation room card on the Consult tab of an open encounter; Today’s Queue calls the patient in and Open chart takes you there.
  2. Consent, then recordRecord consultation asks you to tell the patient it is being recorded and that they can say no; the consent is granted and the recording row hangs off it in the same transaction, so there is no recording without a consent record.
  3. Watch it write itself, or transcribe at the endWhile recording, finished sentences appear as they are spoken over a live session opened under a credential the API mints and spends, and no key reaches the browser; if that session cannot open the recording carries on and Transcribe makes the transcript from the file, and where the organisation does not permit patient data to leave, Transcribe is disabled with the reason printed beside it.
  4. Draft the note and edit itDraft the note suggests chief complaint, history, examination, assessment, plan and advice beside each field — Use this on an empty field, Add to mine on one you typed — and a field’s microphone dictates just that field; Confirm what was said and What might I be missing? are separate reads over the same transcript.
  5. The signature is the clinical actSign into the note accepts the draft with your edits and names who accepted it; only a draft can be accepted, and nothing here writes into the clinical note before that.
  6. AI helpsTurns the transcribed conversation into a draft note for you to sign. You stay in charge.

Rules it keeps

What it does not do, by design. A listing with only benefits is an advertisement.

It does not finalise anything — the signature is the doctor’s, the model version is pinned, and no recording happens without the patient’s consent.

ConsultScribe reads consult audio (consented), templates and writes draft notes, code suggestions. Its data class is Full PHI. Every app works on the same patient record — nothing is copied into a silo.

What happens to our data if we uninstall ConsultScribe?
