What it is

Every patient on warfarin or acenocoumarol enrolled with the target range their doctor set, and every change of target kept with its reason. Each INR result, point of care or from a lab, is recorded with whether doses were missed or there was any bleeding, and keeps the target it was read against. The doctor records the dose decision on each result in their own words, with the date of the next test. The clinic’s list puts first the results waiting for a decision, then tests overdue, then results out of range, and counts how many readings over six months fell in range.
