What IncidentIQ does

Patient-safety incidents reported in two minutes, anonymously if chosen — no reporter is stored on an anonymous report. Severity-matrixed, and closing is a gate: a root-cause analysis on the record and every corrective action done (each with an owner and a due date), or the incident stays open.

What it does not do

It does not show reporter identity anywhere on an anonymous report, and it cannot close an incident over an open corrective action or a missing RCA. A data breach is an incident with deadlines: the server stamps the detection time when it is reported, starts the CERT-In and Data Protection Board clocks from that stamp on the windows your hospital has set, and refuses to close the breach until each notice is recorded as sent or waived with a written reason.
