What it is

CLABSI, CAUTI, VAP and SSI surveillance with denominators from real device-days; bundle compliance and hand-hygiene audits on schedule; two linked isolates open a line list, not a rumour. Ward watch flags an admitted patient’s bed with the precautions and PPE a diagnosis calls for and checks them at the door — who entered and what they missed — so compliance is counted, not assumed; runs each operated patient’s wound watch over the NHSN period (30 days, or 90 with an implant) on the team’s own check days, a classified SSI counting in the rates by surgeon and procedure; and follows an invasive fungal infection in a diabetic or immunocompromised patient from suspicion through imaging, culture and the first antifungal to its outcome.
