What EarlyWarn does

Every vitals entry computes the early-warning score in the open; threshold crossings page a named responder and start a response clock the ward board shows until someone arrives.

How you actually use it

  1. Chart the six observationsRespiratory rate, SpO2, temperature, systolic pressure, heart rate and consciousness — alert, new confusion, responds to voice, responds to pain or unresponsive — with a tick for supplemental oxygen; NEWS2 is charted only on an open admission.
  2. The score comes back computedThe component points, the total and the band are worked out by the service and the schema audits the arithmetic — the total against the sum of its parts and the band against the total.
  3. A worsening band opens an escalationA band of low-medium or worse opens one, and a partial unique index keeps it to one open escalation per admission rather than one per set of observations.
  4. The bell stops honestlyClose escalation needs a responder and a response note saying who reviewed and what changed; an escalation is responded to once, and the response clock on the board is computed from when it opened.
  5. Read the stay’s risk without a third calculatorOpening a row shows the NEWS2 trend beside BedsideRisk’s own Morse, Braden and Padua banding — neither is recomputed here, because a second calculator would be a second answer to the same question.
