What TransitionCare does

The handoff bundle: medication reconciliation against pre-admission lists, first follow-up booked before the patient leaves, red-flag education confirmed by teach-back, summary to the GP.

How you actually use it

  1. Work the queueDischarges without a finished transition lists every discharge whose four facets are not all stamped, oldest first, including the ones nobody has opened — Start it opens the transition against a WardLine admission that is actually discharged.
  2. Stamp medicines reconciledThe stamp carries how many medicines changed; a facet cannot be stamped without its evidence, because the stamp alone is not the work.
  3. Stamp the follow-up and the educationFollow-up booked takes the clinic and the date, and Education given takes what was covered — each stamps once.
  4. Stamp who was told the red flagsRed flags explained takes who was told, the patient or a named carer, and there is no ‘complete’ field to tick because complete is computed from the four stamps.
  5. Log the check callA check call is settled or a problem, and a problem has to be described; the row also shows a readmission within thirty days, read from WardLine and never written back to it.
