What it is

Protocol-driven survivorship: imaging and marker calendars per disease, late-effect screening (cardiac after anthracyclines, thyroid after neck RT), and the plan the patient holds too. After ICU: survivors of critical illness are followed once home for the physical, cognitive and mental-health after-effects in a structured post-ICU review — the ICU stay linked from ICUBoard where it exists (its days and ventilation read, not retyped), reviews on the clinic’s own schedule (set with its source; none ships), what was found in each domain, and named instruments such as MoCA, HADS, IES-R or the six-minute walk cited with the score as scored, never reproduced. Enrolments and milestones are kept on Care Plans, which installs with it.

How it works

Own the protocolA surveillance protocol is a list of visits by month and kind — clinical, imaging or marker — that the clinic edits; a protocol with no visits is refused, and editing one changes what the next enrolment gets rather than the calendars already handed out.
Enrol from the end of treatmentEnrolling takes the primary cancer, the date treatment finished and whether the intent was curative or adjuvant, and the calendar is dated from that completion date rather than from the day somebody got round to enrolling.
Record the late-effect screensA screen records cardiac, fertility, endocrine, bone, psychosocial or second primary, and one marked as referred will not save without the finding it is being referred for.
Raise a suspicion, and let a confirmation end the surveillanceA suspected recurrence has to state what is being done to investigate it; closing it as ruled out or confirmed takes what the investigation showed, and a confirmation closes the survivorship enrolment in the same transaction.
Follow an ICU survivor after they go homeOn After ICU, enrol the survivor from their ICUBoard stay (or a stay elsewhere, as recorded) on the clinic’s own review schedule; each review records what was found physically, cognitively and in mental health, with named instruments cited and scored as scored, and the follow-up closes once, with its reason.
