What it is

The pre-anaesthesia assessment as a checklist with teeth: ASA grade, airway, fasting and medication instructions in the anaesthetist’s words, and a verdict — cleared, cleared with conditions, or deferred with its reason — with the patient nil by mouth on the kitchen’s list from the time set. In the weeks before, a prehab plan in the words of the clinician, physiotherapist or dietician who wrote it, fitness tests as measured and adherence as reported, with a concern raised against the clinic’s own threshold and reviewed by an anaesthetist or surgeon. Then the instructions are sent to the patient on the unit’s own countdown, each message logged with its status and the patient’s confirmation. Child preparation: a child on the surgical list can be prepared beforehand — play, pictures, a book or video, a walk through theatre, practice with a mask — and each session records what was explained, how the child took it, what they asked or fear, what helps them cope and who they want with them, so the anaesthesia and surgical team on the day know what the child already understands. Children listed in the next fortnight show unprepared first.

How it works

Work the clearance worklistEvery upcoming case that has no current clearance, in date order.
Record the assessment and the verdictAirway and the rest, written the way you would say it — "Mallampati II, full neck movement" — then cleared, cleared with conditions, or deferred. Conditions say what must be true before induction; a deferral says why the case cannot proceed yet.
Write the instructions, and send them on a countdownFasting, and which medicines to continue or omit — "continue antihypertensives, omit metformin" — so the ward is not inventing them at 6am. On Countdown they go to the patient in your words on the unit’s own schedule, with what to bring; each message keeps its status, and a call to the patient is recorded as their confirmation.
Run prehab in the weeks beforeOn Prehab — also its own screen for the physio and dietician — write the plan in your words with your targets, record fitness tests by the name your clinic uses, keep adherence as reported, and flag a concern (or let the clinic’s own threshold raise one) for an anaesthetist or surgeon to review.
