What MindNotes does

MSE templates, standardised scales and a risk register with review dates. Sensitive notes carry their own access class within the record.

How you actually use it

  1. Open a draft assessmentFind the patient and press Open a draft assessment. A patient has one unsigned assessment at a time.
  2. Work through the nine domainsPresenting complaint and history, then the mental state examination as nine fields — appearance, speech, mood, affect, thought form, thought content, perception, cognition, insight — a provisional diagnosis and a plan. Save draft as you go.
  3. Grade the riskSelf-harm, harm to others and neglect, each none, low, moderate or high. A moderate or high rating has to be explained in the rationale.
  4. Sign the assessmentSigning is refused until the assessment is whole, and a moderate or high self-harm rating cannot be signed without an active safety plan — the screen attaches the patient’s own. A signed assessment is frozen; write a new one instead of editing it.
  5. Watch the risk listSigned assessments rated moderate or high, with whether a safety plan is attached.
