What it is

MSE templates, standardised scales and a risk register with review dates. Sensitive notes carry their own access class within the record. Process notes are a therapist’s private working notes, apart from the record: read only by their author and the colleagues the author names for that patient, every opening on the author’s own log, kept as written with addenda, and never in a record share, an export, the patient’s own app, the chart, FHIR or the assistant. Medicines follows an adult with ADHD through stimulant titration: each visit logs the dose as the psychiatrist prescribed it, the reason, and the functional response at that visit — the score on the named functional instrument the clinic uses, kept as scored, or the clinician’s own words for work, relationships and daily tasks — an audit trail kept as recorded. Clozapine blood counts and lithium levels are followed against the safe range and monitoring interval the prescriber sets for each patient, with who set it and its source (the national clozapine monitoring scheme or the practice’s lithium protocol, cited by name); results are read from LabBridge or entered with where they came from, and a result outside the range or a test overdue waits on top until a prescriber notes having seen it. Caseload shows a mental-health chain’s counsellors against the weekly capacity the practice sets at each centre, counting in-person and teletherapy sessions the same from TalkTrack, TherapyPlanner and TeleCare, with a counsellor over capacity flagged and each client’s stated preference beside their name.

How it works

Open a draft assessmentFind the patient and press Open a draft assessment. A patient has one unsigned assessment at a time.
Work through the nine domainsPresenting complaint and history, then the mental state examination as nine fields — appearance, speech, mood, affect, thought form, thought content, perception, cognition, insight — a provisional diagnosis and a plan. Save draft as you go.
Grade the riskSelf-harm, harm to others and neglect, each none, low, moderate or high. A moderate or high rating has to be explained in the rationale.
Sign the assessmentSigning is refused until the assessment is whole, and a moderate or high self-harm rating cannot be signed without an active safety plan — the screen attaches the patient’s own. A signed assessment is frozen; write a new one instead of editing it.
Watch the risk list, medicines and caseloadThe risk list holds signed assessments rated moderate or high, with whether a safety plan is attached. Medicines logs each stimulant dose as prescribed against the functional response at that visit, and follows clozapine and lithium results against the range and interval the prescriber set: a result outside it or a test overdue stays on top until a prescriber notes seeing it. Caseload shows each counsellor’s week against the capacity the practice set, in person and video counted the same.
