What HandOver does

Structured handover per patient — situation, background, assessment, recommendations — with open tasks that transfer to the incoming shift explicitly. Nothing is handed over by being forgotten.

How you actually use it

  1. Start a handover for the ward and the shiftOne session per ward per shift, so there is one place the outgoing team is writing into.
  2. Add each patient in SBARSituation, background, assessment, recommendation. The four boxes are the discipline — a paragraph is where things go missing.
  3. Attach what must not be forgottenOpen tasks hang off the patient’s entry and travel with it, instead of living in somebody’s head or on a paper slip.
  4. Hand the shift overHanding over is an act with a time on it, and the tasks that were not done are still open on the other side of it.
