What it is

MDT lists with presentation packets built from the record — imaging, histology, staging — and a recorded consensus decision that lands in the patient’s care plan with a named owner, not in minutes nobody reads. Enrolments and milestones are kept on Care Plans, which installs with it. The breast clinic’s triple assessment sits beside it: a visit for a lump with its clinical exam, imaging and needle biopsy (or the decision that none is needed, with the reason), each recorded as the clinician, radiologist or pathologist reported it with the classification named as they name it, whether all three were done on the visit day, and the combined review read together — concordant, or discordant with a plan — sent on to a board meeting when it needs one.

How it works

Hold the meetingA meeting carries the date it is held and the specialty, and the list shows how many cases it holds and how many are still undecided.
Submit the case with its questionA case takes the patient, a summary and the question the board is being asked — a board answers a question, so the field is not optional.
Record the decision onceDecide writes the decision against a case that has none; a guarded update refuses a second one, because a board decides once.
Send it to a care plan where there is oneNaming a CarePlans template with the decision enrols the patient on that plan and dates its milestones in the same transaction; an unknown template, or one with no milestones on it, is refused rather than silently skipped. A decided case then shows its decision and the plan the patient is now on.
Run the breast clinic’s triple assessmentA visit for a lump takes its exam, imaging and biopsy — or why no biopsy is needed — each as reported, with the classification named as the reporter names it. The list shows what is missing and whether all three were done on the visit day; the combined review, concordant or discordant with a plan, waits until all three are in, and can send the case to a board meeting.
