What it is

Immunohistochemistry and special stains, from the block to the signed report. The blocks come from the specimen’s grossing in SpecimenTrack (A1, A2 and so on, never more than the grossing counted). Ask for a marker panel on one block, such as ER, PR, HER2 and Ki-67 for a breast core. The lab cuts a slide per marker and records each stain run with the antibody lot (picked from LotWatch, where an expired lot is refused) and the positive and negative control. A run whose control failed blocks the read: the slide cannot be scored until a fresh one is cut and stained with both controls passing, and every run stays on record. The pathologist scores each marker in its own system (Allred for ER and PR, 0 to 3+ for HER2, a percentage for Ki-67, positive or negative for a special stain), and the scores go onto the case’s LabBridge report: as an addendum, a new signed version that says what was added, when the report is already signed, or as lines on the report still being written.
