How it works

Log the contact, named or notPhone, walk-in, message or referral, and patient, family, professional or anonymous; choosing anonymous hides the patient link because the row refuses one, and the only mandatory field is what they said was wrong.
Triage in one actUrgency — emergency, urgent or routine — and risk to self on none, low, moderate or high; the grades, the time and the name land together or not at all.
Record what was doneAdvice, appointment booked, emergency services, safety plan shared or referral, each with its detail; actions go only on an open contact.
Chase the follow-upsFollow-ups due is the first card, and each is answered reached, no answer or declined; a no-answer schedules the next attempt in the same transaction, so a call nobody picked up cannot become a contact with nothing due.
Close only when it can be closedClose contact carries resolved, referred or lost to contact, and lost to contact needs a reason; the button says why it is disabled — nothing done this shift on an emergency or high-risk contact, or a follow-up still due — and the refusal itself lives in the guarded update.
