What it is

Pharmaco-therapeutic, infection control, ethics and safety committees, and the board: agendas, quorum verification, decisions recorded, and actions assigned with deadlines that escalate. Board packs put each meeting together in one place to read before and audit after — the agenda with its supporting papers (who prepared each, where it is kept, when it was circulated), the resolutions as proposed and seconded by two members present, counted (never beyond those present) and declared, numbered per committee per year and kept as recorded, attendance against the quorum, the adopted minutes and the actions — and it prints. The risk register keeps every strategic, operational, clinical, financial or compliance risk with its owner and the committee that oversees it, each review’s likelihood and impact on the hospital’s own scale (its size and source set once; none ships) with the score as one times the other and no band attached, the mitigation in words and the next review date the owner sets; a review past its date leads the list, and a risk closes with a reason.

How it works

Constitute the committeeName, kind, the quorum as a number, how many months between meetings and the terms of reference; a live committee of that name here already is refused.
Fill the seatsAppoint takes a member of staff and their role on the committee — chair, infection control nurse, microbiologist; the same person twice on one live committee is refused.
Put it in the diary and hold itMeetings schedules a date, one per committee per date, and Mark as held stamps it; Committees due to meet lists every live committee against the date its own cadence makes it due — the last meeting held plus its months — soonest first.
Work the agendaAdd item, then Record a decision under each one; the Carried forward block is the previous held meeting’s still-open actions re-derived on every load, so closing an action takes it off the next agenda by itself.
Adopt only at quorumAdopt the minutes prints how many were present against this committee’s quorum and stays disabled below it, and blank minutes are refused. On Risk register, set your scale once, enter each risk with its owner, likelihood, impact, mitigation and next review; an overdue review leads the list and each review keeps its score.
