As an old paper chart is digitised, the diagnoses, medicines and key dates on it are pulled out into structured fields against the patient’s electronic record, rather than the chart just becoming a scanned image nobody can search — so a decade of paper history becomes something the clinical system can actually use.
It does not read or interpret the handwriting automatically — a person reads the chart and enters what it says; the app is where that structured entry happens against the patient’s record.
