A prescriber or the pharmacy asks the committee to add a medicine, remove it, restrict it with an approval route, or lift a restriction, and says why. The request is tabled as an item on a P&T meeting in Committees; the decision the committee records there changes the medicine’s listing in the same step, and every change keeps the decision and meeting it came from. A restricted medicine is asked for one patient at a time, and someone other than the person asking approves or declines it. Prescribing off the formulary is never blocked — a doctor may need it in an emergency — and every line written off the list in the window waits on the Exceptions tab until a committee member records that it was reviewed.
What it does not do, by design. A listing with only benefits is an advertisement.
