What it is

The resident medical officer on night duty keeps one log for the night. Each entry is a call attended, an admission taken, a procedure, a death, an escalation or anything else: when, where, who called, the problem, what was done and the outcome, with the patient where there is one. An escalation names the consultant called and stays awaiting reply until their advice is written against it; escalations and deaths are marked for the morning team by default. Only the log’s own RMO writes in it, from the desk or the phone between wards. The night ends with a handover to the doctor taking over, with a note; after that the night does not change. The morning team reads every RMO’s night here, with what is still awaiting a consultant.
