What it is

School and Anganwadi screening for the four Ds — defects at birth, deficiencies, diseases, developmental delays — is logged per child, and a child referred onward to a District Early Intervention Centre is tracked until that referral actually closes with an outcome, not just a referral slip handed over and forgotten.
