What PalliCare does

A palliative enrolment on a care plan, ESAS symptom scores totalled as they are recorded, advance directives — DNR, DNI, no hospital transfer or comfort only — documented against the enrolment with the person who signed them, home visits logged, and opioid prescriptions kept as an append-only record.

How you actually use it

  1. Enrol the patientOn Enrol on palliative care, find the patient, type the primary diagnosis and pick ECOG 0 to 4, then Enrol. A patient has one active enrolment, and enrolling dates the programme’s review milestones.
  2. Score ESASDetail opens the patient. Nine symptoms — pain, tiredness, drowsiness, nausea, appetite, breathless, depression, anxiety, wellbeing — each 0 to 10, then Score ESAS. The total is arithmetic on the server and never typed; fewer than nine or a score outside 0–10 is refused.
  3. Document directivesDo not resuscitate, Do not intubate, No hospital transfer or Comfort measures only, with the patient or surrogate name, then Document. Directives are appended, the latest per kind counts, and the registry row shows them or says no directives.
  4. Log home visitsLog home visit with an optional note. Days since the last visit is computed on every load; a patient never visited says so.
  5. Log opioidsLog opioid with the drug and the dose, appended with its author. Stop asks why the patient is leaving.
