Households and the people in them by village, ward and district, each enrolled in a programme with their consent, and linked to their patient record when they become a patient.
The register a community programme works from. Each household is kept with its head, address, village or ward and district: the same household register HomeNursing’s community rounds walk, so a door planned on a round and a family kept here are one record. The people in it are added one by one with their sex, birth year, relation and phone, and recorded as moved, died or left when they do. A person is enrolled in a programme only with the consent the programme’s own wording asks for: signed, a thumbprint, or given aloud with a named witness, by the person, a parent, a guardian or a carer. The wording is copied onto the enrolment as it was read, so a later change to the programme’s text never alters what someone agreed to. Leaving a programme, withdrawing consent included, is recorded with its date. When a member becomes a patient, they are found in the patient register through its own duplicate check, or registered there, and linked once: the registry never makes a patient record of its own, and one patient is one member.
