What HomeWard does

Hospital-at-home enablement: admission criteria gates, visit logistics for nurses and equipment, RPMConsole-linked monitoring with escalation SLAs, family instructions confirmed by teach-back, and a return-to-hospital pathway that is one call, not a debate.

How you actually use it

  1. Admit somebody at homeFind the patient, then the condition, the escalation hospital, and the carer’s name and phone. The database refuses the admission without both carer fields, and a patient already admitted at home is refused a second episode.
  2. Chart the observationsOpen the admission and enter RR, SpO2, temperature, SBP, HR and ACVPU, saying whether a nurse, carer or device took them. Chart it scores the set as NEWS2 and bands it.
  3. Answer the bellA high band or a red component opens an escalation at the top of the screen with the carer’s phone and the escalation hospital beside it. Close it records who was contacted and the outcome: advice given, visit arranged, ambulance called, or admitted to hospital.
  4. Record the daily reviewType today’s plan and press Record daily review. One review a day is accepted, and the days with none are listed under Days with no review.
  5. Discharge when no bell is ringingDischarge — recovered is offered only when no escalation is open, and the service refuses it while one is.
