What MTPRegister does

Form C/I registers with completeness enforced, opinion requirements by gestation honoured, and access restricted more tightly than any other record class in the platform.

How you actually use it

  1. Admit to the registerThe patient, the date, gestation in weeks (1 to 24), and the RMP giving the opinion. Past 20 weeks the form asks for a second opinion from a different RMP, and the database refuses the same person twice. Each admission takes the next serial number for the year.
  2. Record consentOnce, before completion. For a minor it is recorded as guardian consent.
  3. Complete and sealRefused without consent on the register. Completion seals the row.
  4. Correct by amendmentOn a sealed row, Amend asks which field the amendment corrects, the corrected value and the reason it goes on the register. The row keeps its history: every amendment, its field, its corrected value and its reason, under the admission it belongs to.
