How it works

Admit to the registerThe patient, the date, gestation in weeks (1 to 24), and the RMP giving the opinion. Past 20 weeks the form asks for a second opinion from a different RMP, and the database refuses the same person twice. Each admission takes the next serial number for the year.
Record consentOnce, before completion. For a minor it is recorded as guardian consent.
Complete and sealRefused without consent on the register. Completion seals the row.
Correct by amendmentOn a sealed row, Amend asks which field the amendment corrects, the corrected value and the reason it goes on the register. The row keeps its history: every amendment, its field, its corrected value and its reason, under the admission it belongs to.
