What it is

All intake and output charted against per-condition targets, cumulative balances visible at the bedside and on rounds, restriction breaches flagged as they happen. Lines and charts: each surgical drain’s output and colour at every round (posted to the balance as drain output) trended over 24 hours against the removal criteria the surgeon wrote; each chest drain’s output, air leak and swing, flagged when a leak is still there past the day the surgeon set; a tube feed’s prescribed rate, every change kept, and each shift’s aspirate and tolerance; and the bowel chart — motions typed on the Bristol Stool Form Scale, shifts with none, continence rounds and days since the last motion. A drain or NG tube already on LinesAndTubes is charted as that device and taken out there when it comes out. Nothing ships as a threshold; removal is the doctor’s decision, recorded with their name.

How it works

Open the chart from Today’s balancesEvery open admission with what went in, what came out and the net for today. Entries chart only on an admission that is still open.
Record intake or outputIntake by oral, IV, NG tube or other; output by urine, vomit, drain, stool or other; the millilitres and an optional note. An entry outside 1 to 5000 ml is refused.
Set the daily net targetA floor, a ceiling or both, in millilitres. A target needs at least one bound, and the floor cannot sit above the ceiling.
Read the balancePer shift and per day, computed on every load and stored nowhere. A day outside its target is marked off target in the list rather than written as a flag.
Chart drains, feeds and the bowel chartUnder Lines and charts, pick Drains, Chest drains, Tube feeds or Bowel chart. A drain round’s output counts in the balance; a chest drain adds air leak and swing, flagged when a leak is still there past the day the surgeon set; a feed keeps the prescribed rate and each aspirate check. Removal is the doctor’s call, recorded with their name.
