What WoundWatch does

Standardised wound photos with measurements, charted into a healing trajectory the whole team can see.

How you actually use it

  1. Open the episodeName the site, the etiology — pressure, venous, arterial, diabetic, surgical, traumatic or other — and the day it started; a second open episode at the same site is refused.
  2. Assess it seriallyLength, width and depth in millimetres, the granulation, slough and necrotic percentages, the exudate and a note — each percentage is held to 0–100 and each measurement to its plausible range.
  3. Read the trajectory, not a verdictArea is length by width computed in the SELECT and the trajectory lists those areas against the first one; nothing on the screen says improving, because that is arithmetic for the reader.
  4. Change the dressing by superseding itPlanning a new dressing stamps the current one superseded and inserts the new one in the same transaction, so the episode has exactly one current plan and a readable history of what was tried.
  5. Close it as healed, or say why notHealed takes a date that cannot precede the day the wound started; Close without healing takes a reason instead, and only an open episode does either.
