What it is

eGFR trends stage the disease; checklists carry the patient from CKD clinic to access surgery to the dialysis or transplant list. Kidney clinics: for nephrotic syndrome every dipstick as read by the family or the clinic, the doctor’s own relapse rule (grade, days running and its source; none ships) with a run that meets it listed until a relapse is confirmed, relapses with their remissions and the steroid taper step by step as written; for glomerular disease each biopsy with the diagnosis as reported and any class on the system the pathologist names, proteinuria as measured beside eGFR and the nephrologist’s response on criteria they name; and for a kidney diet the potassium, phosphorus and fluid limits the nephrologist or dietician set with their basis, against each day’s intake, a day over a limit counted.

How it works

Chart the resultChart a result takes the patient, the creatinine and the eGFR exactly as the lab reported it — the equation the lab used is part of the result, so nothing is recomputed here.
Let the stage compute itself, and work the stage 5 listThe registry bands the latest eGFR into a stage on every load and stores nothing, so the day a new result lands the stage moves without anyone editing a field. Every patient in the last band with no fistula recorded is marked and sorted to the top, computed from the readiness stamps rather than set by anybody.
Stamp the readiness checklistVascular access planned, Fistula created, Hepatitis B vaccinated and Dialysis education given are each stamped once with the person who did it, and a fistula cannot be recorded before access was planned.
Keep the transplant workupAdd a workup item for what has to be done, and marking it done stamps who did it — the registry row carries how many are still open.
Run the kidney clinicsOn Kidney clinics, pick nephrotic syndrome, glomerular disease or kidney diet and open a case. Log each dipstick; once the doctor writes the relapse rule (grade, days running, source) a run that meets it is listed until a relapse is confirmed, and the taper is each step as written.
