What PCPNDT Register does

Form F per scan with completeness checks, the statutory register maintained, monthly returns assembled for submission.

How you actually use it

  1. Record the scan on the registerRecord a scan takes the patient, the date it was scanned, the indication from the Form F list — pregnancy dating, anomaly scan, growth, placental localisation or other — and a referring doctor or facility, and ‘other’ will not save until it says what the indication was.
  2. Sign the declarationSign declaration stamps the time and the signing user together on an unsigned draft, and the row then shows that name over a rule with the date under it.
  3. Complete the rowComplete freezes the register row and its guarded update refuses a draft whose declaration is unsigned, so an unsigned scan cannot be closed out.
  4. Amend rather than editOn a completed row, Amend asks which field it corrects, the corrected value and a reason, and appends that as its own row under History — the original text is never overwritten.
  5. Print the monthly returnThe Form F monthly summary counts the register by month and by indication in the SELECT itself, and Print return lays those counts out on paper with a signature line for the person conducting the ultrasonography.
