OnePlan

◦ in design

One care plan every provider sees — coordination with an owner.

Clinical · Full PHI · Every specialty

What OnePlan does

The hub for complex multi-morbid patients: a single shared plan across independent providers, medications reconciled in one place, handoffs as explicit tracked tasks, and a coordinator — professional or family — who sees the whole journey. Cross-organisation sharing rides consented record excerpts, the CrossCare rule.

What it reads

care plansprescriptionsconsents

What it writes

shared planshandoff tasksreconciliations
Same patient record as every other app — nothing is copied into a silo. Data class: Full PHI.

What it does not do

It does not move records between providers without the patient’s consent — coordination is not disclosure.

Every app page carries this section. A listing with only benefits is an advertisement.
