Theatre asks for a frozen section with the site and the question. The clock starts when the tissue is received in the lab and stops at the verbal call, against the lab’s target (20 minutes unless it sets another). The call is logged with what was said, who gave it, who in theatre took it, and that it was read back; a call nobody repeated cannot be logged. When the permanent sections are out, the pathologist records the final diagnosis and whether the call was concordant, deferred or discordant, and a discordance must say what kind (sampling, interpretation, technical) and why. The QA view shows the concordance rate, the deferral rate, the median time and the share within target, with every discordance listed for review.
