What FormWorks does

Intake and screening forms with branching, option scoring and bands. A red flag intercepts submission until a named person acknowledges it with an action. Submitted questionnaires are final; corrections are new submissions against the same version.

How you actually use it

  1. Ask a patient for a form from the To-DoOn the To-Do’s Pre-visit intake section, press Ask a patient for a form: find the patient by name, MRN or phone, pick the Intake form and, if one exists, the visit it is for. Only a form definition in the intake category is offered, and the service refuses any other.
  2. The patient answers it on their portal linkThe request waits in the section until they do. If they have no portal link yet the screen says so and offers to issue one; while it is outstanding, Withdraw takes it back with a reason.
  3. A red flag stops on the To-DoAn answer that trips a red flag appears under Red flags from forms until somebody presses Acknowledge with action and says what was done — that is recorded on the submission.
  4. Draft a history from their answersWhen the form comes back it lands under Answered. Press Draft a history from their answers, then edit the draft in the box — what you accept is what goes into the note.
  5. Accept it into a draft noteAccept into a draft note needs a visit — pick one from today’s list if the form was asked before one existed. The note is created as a draft you sign yourself in the notes screen; nothing here signs anything.
