What it is

A root canal treatment for one tooth, charted at the depth endodontics needs. Each canal (MB, MB2, DB, P, and so on) carries its working length in millimetres with the reference point it was taken from, how it was measured (apex locator, radiograph or both) and the master apical file. Visits record what was done — access, shaping, irrigation, the medicament placed, the temporary seal — by whom and when. Obturation is refused while any canal has no working length, and records the technique and sealer; after it the canals are frozen, so a re-treatment is a new case rather than an edit. The dentist sets the date the crown is due; an obturated tooth past that date shows on the list until the crown is recorded. The obturation and the crown are written onto ChairSide’s tooth chart in the same step, and the review radiograph is recorded with the dentist’s note. One open root canal per tooth per patient.
