What it is

A view inside Discharge (DischargeLine) — installing it installs DischargeLine. Every open admission listed with the facts that bear on going home — another admission in the window the reader chooses, no future appointment booked, no phone number on the record, an overdue care-plan milestone, an overdue recall, no discharge summary written — each naming the row it came from, ordered by how many are true. These are facts, not a score: a readmission risk model is a published, validated instrument with coefficients; none is loaded here, and inventing weights so a percentage could be displayed would be worse than showing none.
