What it is

Each radiotherapy course with the prescription the radiation oncologist entered — the site, the intent, the total dose and the number of fractions, and whether chemotherapy runs alongside. Each fraction is recorded by the treatment room as it is delivered, numbered in order and never past the number prescribed; a course on hold takes no fraction until the oncologist resumes it. Every interruption is kept with its reason and its days — a machine down, a patient who did not come, a hold for toxicity — so the overall treatment time and the gap days are on the course, not worked out later. Once a week the oncologist reviews the patient and grades each toxicity they assessed on CTCAE v5.0 terms, with the decision to continue, hold or stop; a hold or a stop takes effect with the review. A course on treatment with no review in seven days is shown overdue.
