What HazMatLog does

Spill-kit locations and checks, MSDS access at the point of need, and needle-stick/exposure incidents with the PEP protocol started on the spot and followed to serology. Each exposure also shows on that person’s staff health file in StaffHealth, read from here and never copied.

How you actually use it

  1. Report an exposureNeedlestick, splash, chemical spill, radiation or biological; the staff member exposed, from the roster — the form has no patient field; when, where, the substance or source, and what was done immediately. A needlestick or splash records the source patient’s HBV, HCV and HIV status — positive, negative, unknown or not tested.
  2. Work the prophylaxis clock, and record the decisionThe list at the top is every needlestick or splash still inside the 72-hour window with no decision recorded, with the hours elapsed and left computed at the moment of the request and stored nowhere. Record it as offered and started, with the regimen and when the first dose was given; offered and declined, with the refusal form signed; or not indicated, with the reason a reviewer can agree or disagree with.
  3. Record the follow-up serologyBaseline and weeks 6, 12 and 24, each with the date drawn and the result; an interval still undrawn past its due date is marked overdue.
  4. Close the exposureWith a closing note. A needlestick or splash cannot close until a prophylaxis decision is recorded — not indicated with its reason counts.
  5. Keep the spill kitsRegister each kit by location and how often it is checked, then record every check as complete, or incomplete with what is missing. Overdue checks are marked.
