What NudgeList does

The private follow-up scratchpad: "check on this patient in five days", flagged in two taps during the consult, nudged on the day, cleared with one. Single-doctor, zero setup — the worry list that currently lives in memory and gets lost there.

Every record stays; only the workflow leaves.
