A patient or caregiver doing peritoneal dialysis exchanges at home logs each one — volume in, dwell time, volume and clarity out — and the training that qualified them to do it alone is signed off and kept on file, so the unit can tell a missed exchange from a fluid problem.
It does not perform or supervise the exchange — it is the log and the training record; a nurse still trains and periodically reassesses the patient.
