What it is

A patient with recurrent fainting has each episode logged with its circumstances, tilt-table test results tracked against the episodes, and the driving-fitness advice given recorded, so the workup toward a cause — cardiac, neurological, or reflex — has one continuous record instead of restarting at each new specialist.
