What it is

A pulmonary hypertension patient’s six-minute walk distance, WHO functional class, and drug titration are tracked together visit to visit, the three measures that together decide whether therapy is working, escalating, or failing — read separately, each one alone can miss a patient who is quietly deteriorating.
