What it is

Glasgow Coma Scale and pupil checks recorded on the hour for a head-injury admission, trended on one chart so a two-point drop across three readings is visible immediately instead of buried in a paper obs sheet. A deteriorating trend escalates to the covering doctor rather than waiting for the next round.
