A cochlear implant patient’s journey — audiology candidacy testing, the surgery itself, and the mapping sessions that follow for years afterward — is kept as one record, because mapping visits are frequent early on and then spread out over a lifetime, and a gap in that record makes it hard to tell if a child’s device is still correctly tuned.
