Every kangaroo mother care session — who held the baby, how long skin-to-skin lasted, and the baby's temperature and behaviour during it — logged against the newborn's record, so a NICU or postnatal ward can show how much KMC a baby actually received, not just that it was 'encouraged'.
It does not replace vitals monitoring — it logs the session; a separate monitor or nurse still watches the baby during it.
