A patient's advance medical directive — the treatments they do and do not want if they cannot decide for themselves — held on their record where an admitting doctor or an ICU team will actually see it, with the date it was made and by whom it was witnessed. A later directive supersedes an earlier one automatically the day it is filed.
It does not adjudicate a family's objection to the directive or make a treatment decision — it puts the patient's own written wishes in front of the clinician who has to decide.
