Every surgical admission gets a Caprini score at the point of admission, and the score maps to a recommended prophylaxis — mechanical, pharmacological or both — so a VTE risk is scored the same way for every patient rather than only for the ones somebody remembers to think about.
It does not order the prophylaxis itself — it scores the risk and shows the recommendation; the admitting doctor orders it.
